Kaiser Santa Rosa is pleased to announce its 2008 Community Benefit Grant Program. For sixty years, Kaiser Permanente's social mission has been the foundation for its community service programs. As a values-driven, not-for-profit integrated health care program, Kaiser Permanente is dedicated to improving the health of its members and the communities it serves.
Through partnerships with community organizations and government agencies, Kaiser Permanente addresses issues and concerns that affect overall community health. These partnerships are built upon the sharing of knowledge and resources between Kaiser Permanente and health-related community service organizations. Kaiser Permanente brings a variety of resources to these partnerships: cash grants to support community activities, volunteers and in-kind donations.

Each year, Kaiser Permanente provides funds to community organizations and government entities to improve the health of the communities we serve. Kaiser Santa Rosa has identified priority health and social needs that impact the lives of vulnerable populations through a community needs assessment that is conducted every three years. These priorities provide the basis for the Kaiser Permanente Marin / Sonoma Community Benefit Program and guide the distribution of Kaiser Permanente Santa Rosa Medical Center contributions to the community.
The priority needs identified for the Kaiser Santa Rosa area through 2008 include:

Applications are accepted February 1 to March 3, 2008. The application can be downloaded beginning February 1.
• Promote improvement in children's health status by addressing obesity through healthy eating and active living; improving oral health; reduction of teen alcohol, drug, and tobacco use; and reduction of perinatal substance use.
• Support Community Health Centers and other Safety Net providers of care for underinsured and uninsured.
• Support programs which increase access to care and coverage, as well as supportive services, for vulnerable populations.
• Promote prevention services to improve the health and well being of people living in Sonoma County.
The Community Benefit department is accepting applications for our 2008 Community Benefit Grants program February 1 to March 3, 2008. The grant application will be available from this site beginning February 1. Requests from $5,000 to $20,000 will be considered.
