Your First Visit

New or first time patients are seen by referral from your personal physician or nurse practitioner. You may be asked to fill out a patient questionnaire. If you receive one, please bring it with you to the visit. While we are able to review your laboratory reports, medications filled at the Kaiser pharmacy and recent imaging studies on our computers, we do not have access to non-Kaiser medical systems. For this reason, please make sure we receive copies of all records or x-rays taken at other facilities that pertain to your current problem. It is often helpful to bring a list of specific questions and concerns you may have to each visit.
