How to Leave an Effective Phone Message

1. Speak slowly and clearly.
2. Provide your name, Kaiser Permanente patient number, and telephone number.
3. Provide the physician that is providing your care.
4. Make your message concise and to the point

How to Get Timely Medication Refills

1. Call well ahead of time. Don't wait until your prescription has run out!
2. Be aware of how many refills you have remaining.
3. Better to call and leave message, than showing up at the clinic window.
4. Speak slowly and clearly.
5. Provide the prescription refill information. (See above.)

How to Schedule a Return (Established Appointment)

1. Call the clinic! (707) 566-5557
2. State your name, Kaiser Permanente patient number and the physician providing your care.
3. Provide the reason you require a follow-up appointment.
4. If you are leaving a message, be sure to provide a phone number where you can be reached.

How to Get a Referral to the Department of PM&R:

1. Talk to your primary care provider.
2. Your personal physician at Kaiser Permanente can generate a computerized referral to our department.
3. Our department will contact you.
