Health EducationWe have two health education centers in Santa Rosa to serve you, one on our main campus at 401 Bicentennial Way (707) 393-4167 [Map] and the other at the Stein Medical Office Building, 3925 Old Redwood Highway (707) 566-5277 [Map]. The health education centers are a wonderful place to find a variety of health resources. We provide our members with healthy living information on many health topics in the form of videos, DVDs, audiotapes, CDs, books and pamphlets. We even offer computerized literature searches, all for free. Some of the health topics that are of particular interest to our senior members are: aging & senior health, chronic pain, death and dying, health, wellness & fitness, mental health, neurology, and nutrition, among others. You are invited to use the health education center as a quiet place to read or watch our videos and DVDs. You can borrow books, videos, DVDs, CDs, and audiotapes from the center, or purchase books at a discount to members. Health Education ClassesBrowse the class catalog and class descriptions. Healthier Living With Ongoing Health ConditionsLiving with an ongoing health condition such as diabetes, hypertension, or arthritis doesn't mean you have to be ruled by it. This interactive workshop can help you manage the challenges you may be facing. Learn smart strategies to more effectively manage your symptoms, fight fatigue and frustration, make daily tasks easier, and get on with your life. Open to the community. 6 sessions. Advance Care Planning![]() Talking with your family, friends and your physician about your wishes for medical care at the end of your life is called advance care planning. It's a way for you, your loved ones and your physician to discuss the kinds of care you want and don't want at that time. You can also specify the care you would want if you become unable to speak or make decisions for yourself, due to a coma or other medical condition. Advanced Health Care Directives ClassMake sure your wishes will be respected if you become too ill to make your own health decisions. In this two-hour class, you'll explore the legal forms and learn what to consider when delegating your health care decisions. You may bring a support person. This class is taught by a local attorney and a Kaiser Permanente clinical health educator. It is co-sponsored by Seniors at Home, a non-sectarian program of Jewish Family and Children's Services, and attorneys Alice L. Perlman and Janice Sternfeld, Perlman & Sternfeld, LLP. Mental, Emotional and Behavioral HealthAdult PsychiatryThe department of adult psychiatry offers a wide variety of outpatient services for adults who live with psychiatric disorders. You do not need a referral to the department to be screened for services. If you are interested in an intake appointment, you may call the department directly and a member of our triage team will contact you within 24 hours for a short phone interview and to schedule an appointment to meet you in person. Memory Disorders Clinic![]() Patients and/or caregivers are invited to attend an educational meeting to learn more about memory disorders in general. Following this overview program, each patient and his/her family member or caregiver may see a physician or nurse specialist for individual history, testing, diagnosis, recommendations and treatment. To make an appointment, ask your medical provider for a referral. |
Behavioral MedicineA behavioral medicine consultant (BMC) is present on each of the adult medicine modules and works closely with your doctor or nurse practitioner. The BMC can help you learn to cope with the everyday stresses that affect your health and contribute to illness or physical symptoms. These stresses could include: living with an illness or painful condition, problems at home or work, the loss of a loved one, depression or anxiety, or the challenges of caregiving. He or she can offer you information and recommendations regarding the full range of Kaiser Permanente services available to members. Location: Santa Rosa and Rohnert Park Medical Offices

Chronic Condition ServicesType 2 Diabetes Support GroupShare your experiences and learn from others with Type 2 diabetes. This monthly drop-in support group is led by a behavioral medicine consultant. Occasional guest speakers are invited to discuss a variety of related topics. There is no co-payment for this group. Physical Therapy ClassesBalance Class Part 1 and Part 2![]() Learn and practice exercises you can do at home to improve both your balance and strength for everyday activities such as walking, getting out of chairs or going up and down stairs. A physical therapist will observe your walking and additional instruction will be given to those using a cane or walker. Class handouts include tips for improving home safety to reduce the risk of falls. Suitable exercise clothing is recommended. Each class lasts 90 minutes Location: Santa Rosa

Fall PreventionMost falls are preventable. In this one session (90 minute) class, participants will learn about common causes of falls in and around the home and make a plan for one or more specific changes they can make to improve their safety and reduce their risk of falling. Class will also include a demonstration of assorted safety devices. Family members and care givers are encouraged to attend together. No referral needed. Free to all members. Location: Santa Rosa |
Chronic Conditions Management![]() Case Management is offered to qualified members with chronic health conditions. The goal is to educate and support the person to successfully manage his or her own care, to reduce risk or slow disease progression and potential complications. The member and his or her support system actively participate to develop goals and work toward them. Case management programs are available by physician or nurse practitioner referral only. For more information: Ask your physician if you think this service might be of benefit to you. Coordination of Care (Social Work Services and Discharge Planning)Provides social work services and discharge planning to hospitalized members. These may include referral to home care and nursing care facilities after hospitalization; information and referral to other community social services, and supportive counseling in the hospital. The goal of the department is to assist members continue their recuperation, to remain safe after their discharge from the hospital and to be able to access supportive Kaiser programs and other services in Sonoma County. Consultation about care resources in the community is available for outpatients by referral from your physician or nurse practitioner. |
